Single-sided deafness (SSD) occurs when a person has little to no usable hearing in one ear and normal or near-normal hearing in the other. This condition can develop suddenly or over time. Some cases are linked to viral infections, acoustic tumors, head trauma, or sudden sensorineural hearing loss.

Even though one ear hears normally, SSD can still cause significant daily challenges. Hearing with one ear affects the ability to locate sounds, understand speech in noise, and maintain listening comfort. Many adults are surprised by how much impact losing hearing in one ear can have.

How SSD Affects Daily Life

Pinpointing where a noise comes from is one of the hardest parts of living with SSD. Usually, your brain compares how loud a sound is in each ear to find its source. When you only hear on one side, that system breaks down. It is like a constant glitch that makes the world feel confusing and out of sync. Following a conversation in a noisy room is also a major struggle. If background noise hits your “good” side, your head actually blocks other sounds from reaching your ear. 

The Emotional Impact of SSD

Living with SSD can be very lonely, especially in big groups. Many people find themselves constantly moving around just to hear better, or they might just stop going to busy places altogether. It is hard to feel part of the conversation when you are always struggling to catch what others are saying. The world can even feel a bit scary when you cannot tell where sounds are coming from. Crossing the street or driving becomes stressful when you are not sure if a car is near you or far away. Because of this, it is so important for doctors to really listen and understand these daily fears when helping someone with hearing loss.

Diagnostic Evaluation

A full hearing evaluation will confirm the degree of hearing loss in each ear. In cases of sudden hearing loss, urgent medical referral is often required to rule out any serious causes.

Imaging, like an  MRI, could be recommended to rule out an acoustic neuroma or some other structural condition. Identifying the cause of SSD helps guide appropriate treatment planning. Speech testing in quiet and noise also provides valuable information about how the better ear is functioning under stress.

Treatment Option: CROS Hearing Systems

One common treatment for SSD is a Contralateral Routing of Signal (CROS) system. This system uses a microphone worn on the deaf ear to pick up sound and transmit it wirelessly to a receiver in the better ear.

CROS systems do not restore hearing to the deaf ear. Instead, they reduce the head shadow effect by allowing sounds from the poorer side to reach the functioning ear. This can improve awareness and communication in certain environments.

Treatment Option: Bone Conduction Devices

Bone conduction devices are another option. These devices send sound vibrations through the skull directly to the cochlea of the better ear. They can be worn externally or surgically implanted, depending on patient preference and candidacy. Like CROS systems, bone conduction devices help improve sound awareness from the poorer side but do not restore true binaural hearing.

Treatment Option: Cochlear Implantation

In some cases, cochlear implantation may be considered for SSD. Unlike CROS or bone-conduction systems, a cochlear implant directly stimulates the auditory nerve in the deaf ear.

For appropriate candidates, cochlear implants may more effectively improve sound localization and speech understanding in noise than routing systems. However, outcomes vary, and careful evaluation is required.

Long-Term Follow-Up

Adults with SSD benefit from follow-up appointments to assess device performance and adjust settings as needed. Listening demands may change over time, especially with any work or lifestyle changes. Regular monitoring also makes sure that the better ear remains healthy. Protecting the hearing ear is vital, as it carries the full responsibility for sound detection.
